BJC HospiceThe BJC Hospice program was developed to support patients and families in coping with life-limiting illnesses while living life to the fullest. Care continues after the death of the patient to support families in their grief. The hospice team includes: registered nurses, counselors, physical, occupation and speech therapists, home health aides/homemakers, chaplins, registed dietitians, medical social workers, volunteers and bereavements specialist.

Pathways Community Hospice
Pathways Community Hospice provides patients the opportunity to spend the remaining months of life's final journey in familiar surroundings...alert and free of pain...among the people and things they love. We are dedicated to making this possible. Pathways Community Hospice is a Medicare/Medicaid Certified hospice that works closely with the patient's physician to ensure coordination and continuity of care.
Services are provided in the patient's home or nursing home and include: Professional nursing and state of the art pain and symptom management; Coordination of medications, medical equipment and supplies; Hospice aides for personal care and hygiene; Skilled counseling for end-of-life issues; Occupational, Speech and Physical Therapies, if needed for palliative care; Medical Social Worker; Spiritual Care; Trained Volunteers for family respite and assistance; Bereavement Program; Coordination of short term inpatient care for pain management and symptom management and for respite care.
